Question: Sample email

I need to send something out to my current Committee Chairs to see who will be returning and who will be leaving. I also want to get and update description of what the committee does and get details - how many volunteers needed, budget, roadblocks, etc. so I was thinking about creating a five or six question survey. Does anyone have a sample email or survey with similar information?

We don't have a survey for this exact purpose -- and it's a great idea! We do have a survey, however, that ask for feedback from committee chairs after specific events. This could be a good start for you to craft an email to your chairs:
