I am in need of a teacher survey. Does anyone have any examples they would be willing to share. We are basically asking, how can we help you? / how can you help us? If you don't mind sharing you can post here or email me direct -

You can find lot's of good ideas by doing a search on this web site. Many samples for parent or teachers. Hope this helps.

I'm just curious. How many of you did send the survey home, and what kinda response did you get?
In other words, if you sent it home to 500 famlies, did you get at least 100 back, or 25 back, or more?
Lots of great stuff can be gathered from surveys, but I know back when we did ours at the elem school - 6 yrs ago - yikes - we got a very low response.

We sent out about 300 and we got about 100 back. We did get some good feedback and got to see what people are interested in.

We've done an end of the year survey several years. The one year that comes to mind, we got 114 back. There were about 500 kids, probably 300-350 families.
We always tie ours to a drawing, like a Toy's R US gift certficate. You get the best data if the survey is anonymous, so we would trade a completed survey for a drawing ticket. Then the child could fill out the ticket and put it in the jar. The jar was always in a nice, visible place and we had signs promoting the survey.
