Hi, I'm like many of you, Newer member of a PTO/PTA group. I finally got the board to agree that we need to send out volunteer forms, parent surveys or questionaires. To see how we are doing in the eyes of the other parents. I personally like to see what kind of response we receive. I'd like to ask anyone who uses any of these type of form to please submit them here. I would really appreciate the help. I will be glad to pass them along. Hello! In California each PTA President is given a PTA toolkit which had lots of information such as sample surveys, forms, sample letters ect. You should call your state PTA and see if they have such a thing if not I will copy the sample that I have and email the document to you. TOCS PTA MOM
