I am looking for a form to use when someone is coming to the PTO to request financial support for an extra project, etc. that is not part of the PTO's annual budget. This would be the first step someone would use when wanting to get financial support from the PTO, before they actually come to a meeting to share the need.

We have a surplus from last year that we will be disbursing and looking for a good way to handle it.
