This is a form that our PTO used to see where we can get parents involved. We used this form to see where parents want to volunteer, get parent contact information, find out why they do or do not attend PTO meetings, and what can we change to get more parents involved. We also awarded the class with the most forms returned with a Pizza Party to try to get the kids to take them home and have them filled out compleated.
