Fill in the budget set aside for that item, then give this sheet to the chairperson of that event. The chair person or action person should then use the form to record the items needed, who is responsible for getting the items, and when the items have been secured. There's also a section for committee members' names, contact info, and a schedule for larger events like spring carnival. It is set up so that you can type info into the spaces, but it can be printed off and used as a master sheet.
