Short Description:	2 half sheet membership forms on one letter sized page
Long Description:	This form works well after the beginning of the year. I recommend that you replace the top form with a letter about your school and all the wonderful plans you have for the year for any distrbutions that will be taken home. I'd ask permission of your principal to have this inserted with their forms at the beginning of the year.
