This form was used to allow the students to dictate how they wanted their prize entries to be distributed. Depending on the amount donated, the student got anywhere from 1 to 20 prize entries. We allowed the kids to decide for themselves how to split their tickets among the four "big prizes". We did not have one automatic prize for the highest donor. We accepted empty donation envelopes and gave one raffle entry for any received by our deadline. We had 25 participation prizes (one entry for each participant). We gave all the prizes away at the school dance.
