Our previous coordinator created the original and I have modified it for the current school year. The 8.5 x 11 page is divided into four small instruction sheets that give info on the programs and have a place for student name, teacher, and grade (useful for tracking with contests). We print these out, cut into quarters, then staple a sheet to a single sandwich-size Ziploc-style baggie. These may be used in addition to or in place of collection sheets. The returned baggies are recycled when we bundle the Box Tops to mail in.
