Permission Slip for Jog-a-thon

One of our PTO fundraisers is a jog-a-thon held each spring. We were wondering a) Do we need to have the parents fill-out a permission slip to allow eachi student to particpate? b) How long do we keep permission slips? Thank you.

We are looking to do this type of event and are holding the event during gym class, so I owuld imagine that if you are doing it during school hours, you would not need one. If you are doing it outside school hours, I would imagine you have to seek parent permission and you would need need to keep them through the event and after (in case of liability). Hope that helps...
