Post-Event Feedback Requests

Have any of you requested parents/families to fill out a survey regarding the event they just attended? I'm looking for a "form" to provide our families with. Do you give the survey to all families even if they didn't attend and ask why or just those that did attend?

That seems like a great idea! We don't currently have an event evaluation form in our File Exchange, but we will put a question out on the message board to see if someone would like to share one.
