Removal of officer - written policy

Since it is a volunteer position. Can you help with writing a policy to remove an officer if they are not doing the job or because of ethical issues?

The executive board may, by two thirds affirmative vote, remove any officer or chairman for failure to perform duties, unethical behavior in PTO business, and being absent for three consecutive meetings of this PTO.
