This form has space for two adult volunteer names and then a column for each to check the areas in which they might help.

It was an attempt to build a master database (Excel spreadsheet) that would be used all year. Moms still fill most volunteer slots, but this gave us information on the dad - or other adult.

(Today's families come in a lot of formats - can't assume "mom" and "dad". This one has "adult 1" and "adult 2".)
