Conflict of Interest Policy and Agreement
The first 3 pages the board normally approves at a board meeting. It is then signed by the Pres/VP. The last page each board member signs in acknowledgment of the agreement. All of the original signed copies are kept together. This should happen each year, assuming members on the board change.
