when we did ours we actually just took ten things and asked them to put them in order of the importance that they see. We had things like volunteering, classroom donations, contributing to field trips, class parents, etc. It was nice to get the results because they all placed the same items on the top and then we knew as a group how to help them the most. There was a spot for suggestions as well.

Thanks so much for your responses. I agree with OneandOnly that alot of survey probably won't be returned, but I like the idea adding the wish list. Thanks so much!

We do a 'wish list' that we post on our website so parents can donate items for the classrooms. We send out a survey asking the teachers to think of anything they want for their room or the school in general. We also include any questions we might have that year and a space for the teachers to tell us any concerns they have. We get all of these surveys back since the wish list catches their eye.
