Long Description:	Get (or guilt) your family into collecting those Box Tops by sending a personalized collection sheet with your child's photo. No one can throw away a photo of their beloved grandchild, niece, nephew, godchild, etc... :-) Spaces for 25 Box Tops. One set of my son's grandparents live at the beach, so this sheet is catered to the beach scene. But you can change the clipart and headings to fit your needs. A pretty basic form, but the photo gets them collecting every time.
