Spend the money to buy the PTO Today start up toolkit; it's got a sample form 1023 in there that you can copy from. Then make sure you follow the directions here: www.irs.gov/pub/irs-drop/n-11-43.pdf
because it sounds like you will probably qualify for the transitional relief program so that you won't have to pay as big a user fee.
When you say you "barely make 5k each year," you mean gross, not net, right?
Has anyone had their 501c3 revoked? Just found out...apparently, tax returns haven't been filed for the past three years (we barely make 5k each year). Now trying to figure out how to fill out Form 1023 to get reinstated.
