I also want to put on the survey what the students feel the school or their classroom needs.

My youngest son's elementary does an exit survey when the fifth graders are leaving the school for middle school. Is that what you're looking for? We also do a very simple one for the whole student body that is mostly a rating thing like on a scale on 1 to 5 what did you think of 1) The author visit 2)Earth Day 3)Day of the arts
and then we ask about how they liked the fundraisers and if they have any suggestions for different ones. It only takes about 5 minutes to fill out and if I remember correctly, it can be anonyomous. Oh, and we always ask about when the PTA meetings should be and half a dozen people say it should be at night (we have morning, afternoon and two night meetings) but those people never come to the night ones we have anyway. My suggestion is to make it very simple but have room for comments. 90% don't comment but the 10% that do are very interesting!

I need a sample of a student survey. I was wondering if anyone has done this for K-8. I am not sure how to do the younger classes but I want to include everyone.
