I am also a new President. What we have been doing for several years is a PPO membership drive. We have a printing company make envelopes for us. On the front they say:
PPO membership only $5 per family
Then it asks for
1. the name of the person who wants to join
2. their address
3. best way to contact
4. email address
5. how would they like to help
Then at the bottom there is a table for their child/childrens information.
We leave spaces for 5 children because sometimes we have grandparents that join and they have several grandchildren in our school.
The information we request for the child/children is:
name, room number, teacher, and grade level.
The reason for this is because if for some reason the parents contact information changes then we can always resort back to a letter home with the child.

just emailed them to you. I did what pzettler suggested and had the files/forms on my computer and made a disc for the new pres. and then made another one when that one failed to pass on the disc when she left.
Welcome to the two new PTO presidents. Just a couple of tips that may help...
Go buy a big fat binder and a three hole punch. When you get the letter and membership form you like, put it in the binder. You'll already be more organized than the past president.
Make a folder on your computer for your PTO. Save all the stuff you like in that folder. You are now ready to alter the letter for use next year. If you are not re-elected, then go buy a thumb/flash drive and copy all the items over for the new president.
