I was looking online and may have found something that can work:

Form .

It is a pdf file but does open as a Word Document so editing the document to fit your organization will be extremely easy. On the top, it does have the logo for the form's company, but that can be easily deleted and replaced with something else. If you are having problems opening the document in word, just let me know and I can email the form directly to you. Hope this helps!

I am looking for a generic membership form.... we don't charge fee's for being a member, and so I am just looking for something simple..

Also, I am working on contunity binders for each board member... Does anyone have something like this??
