OUR PTO HAD AN ELECTION AND NO VICE PRES WAS ON THE BALLOT OR ELECTED. WHO AND HOW IS THE POSITION FILLED?

The practical answer is board members recruit someone to fill the roll. That person serves on an interim basis until you next meeting (which may be next year). At the next meeting after you've gotten someone to accept the job, you hold an election just for the office of vice president.
